Intrapericardial bronchogenic cyst with marked production of antigen CA 19-9; a case report.
The Journal of the Japanese Association for Chest Surgery, 1998 症例は33歳女性, 前胸部痛, 咳嗽, を主訴に当院受診.画像診断にて右前縦隔に心臓を前方より圧迫する10cm程の嚢胞を認めた.末梢血のCA19-9は12,531U/mlと高値を示しており, 嚢胞内液のCA19-9は240万U/ml以上と異常高値を示していた.手術所見では嚢胞は心嚢内に進展していたが直接浸潤は認めなかった.病理所見では嚢胞壁は多列繊毛上皮に覆われ気管支腺, 軟骨, 平滑筋, 肺組織も認められた.術後CA19-9は低下し正常化した.気管支性嚢胞は縦隔内のいたるところに発生するが, 心嚢内に進展した症例はきわめて稀である.また嚢胞内液のCA19-9が本例のような異常高値を呈した症例は報告が無い.
