Treatment of Chronic Anal Fissure -Anal Dilatation-

慢性裂肛の治療について我々の施設で施行している用手肛門拡張術を中心に述べた．用手肛門拡張術は麻酔下で手指を肛門内に挿入して肛門を拡張する方法であるが，その長所は手技が簡単で創を作らないことであり，侵襲が少ないため外来手術にも適している． 我々の施設では2010年の1年間に170例の裂肛に対して用手肛門拡張術を施行し，有効率は90.6％であった．合併症は3.5％発生したが，いずれも軽度であり保存的治療で改善した．手技は術者の感覚によるところが大きいが，愛護的に行えば安全性が高く，肛門内圧の高緊張状態を速やかに改善するので適応範囲も広く，保存的治療に抵抗する慢性裂肛に対して第1選択になり得る方法と考えられる．
