The differential diagnosis also includes ACE inhibitor therapy, pertussis, and, in up to 80% of patients, multiple causes. Response to treatment may offer diagnostic confirmation but can be slow in coming.

In non-smokers without intake of an ACE-inhibitor, the three most common causes of chronic cough are eosinophilic airways disease (asthma or eosinophilic bronchitis), Upper-airway-cough-syndrome (UACS) and Gastro-esophageal-reflux desease (GERD). In smokers, chronic bronchitis and COPD are common causes as well.
