Nutrition in Clinical Practice, 1996With growing concerns over cost containment, hospitals are using clinical pathways to standardize health care and reduce costs. Clinical pathways designate the actions and services that patients should receive at specified time intervals throughout the hospital stay.

Charting by Exception on a Clinical Pathway
Nursing Management (Springhouse), 1997A study at one medical center showed that nurses were spending 2.5 hours per shift documenting patient care. After charting by exception was integrated with a clinical pathway, charting time dropped to 0.82 hours per shift. Results of a pilot project on an orthopedic/neurologic patient care unit are discussed.
