症例は78歳，女性．平成15年4月，膵頭部に3cm大の腫瘤像を認め分枝型IPMNと診断され，以後外来で経過観察されていた．平成18年5月，下腹部痛，微熱が出現し，当院に入院となった．CT上，腫瘍の増大傾向を認め，嚢胞内に新たに充実性病変を認めた．ERCP検査にて主膵管は数珠状に拡張し，嚢胞内に充実性成分と思われる欠損像を認めた．膵管内超音波検査にて嚢胞内に充実性の腫瘤を認め，生検ではIPMN with moderate dysplasiaであった．ERCP5日後，閉塞性黄疸を認めたためERBDチューブを留置した．減黄後，膵頭十二指腸切除術を施行した．術後の標本検索では，嚢胞と総胆管に直径約1cmの瘻孔を認めた．病理検査上，瘻孔部に腫瘍細胞は認めなかった ...
