目的: 頸部腺癌が細胞診によるスクリーニングから漏れる原因を明らかにすること.方法:当科で治療を受けた子宮頸部腺癌59例の臨床的特徴, 細胞像の特徴, 腫瘍性背景の出現頻度を検討した.成績: 1) 臨床進行期の内訳は, 0期3.4%, Ia期11.9%, Ib期50.8%で, 扁平上皮癌に比し, 早期癌が少なかった.2) 1年以内の検診歴がIb期3例, II期4例にあり, 浸潤癌になっても細胞診断上の確定診断が困難な症例があった.3) 細胞所見では, 重積するクラスターの出現は必ずしも認められず, 核異型も弱いことが多く, 最も高頻度に認められた異常は, 配列の乱れと核の増大, 大小不同だった.4) 細胞診における腫瘍性背景の頻度は, Ia期14.3%, Ib期34.5%, II期60.0%で ...
