Dermatitis herpetiformis (DH) is a chronic, polymorphic, pruritic skin disease that develops mostly in patients with latent gluten-sensitive enteropathy. DH patients usually present with skin manifestations only and are not aware of the underlying small-bowel problems.

Dermatitis herpetiformis (DH) is an immune-mediated blistering disorder associated with gluten hypersensitivity and characterized by deposition of immunoglobulin A (IgA) in a granular pattern at the dermoepidermal junction. Although it may present at any age, onset is most common from early adulthood to the fifth decade of life.

To the Editor.— Despite the unusual feature of scarring, the case reported by Trepanier 1 may well be one of localized bullous dermatitis herpetiformis. Certainly its histopathology, chronicity, pruritus, and response to sulfone therapy speak for that diagnosis.

Observations have been made with respect to the types of cutaneous lesions which are apt to predominate in patients with dermatitis herpetiformis at certain age levels. It has been brought out by Brocq1that the peaks of incidence in dermatitis herpetiformis apparently fall at about the age of 20 and at about the age of 60.
