New treatment of chilblain. Using diltiazem hydrochloride.
Nishi Nihon Hifuka, 1987凍瘡は日常しばしば遭遇する疾患であるが, きめてになるような治療法は現在みつかつていない。Ca拮抗剤(塩酸ジルチアゼム)の末梢血管と血液性状に対する作用に注目し, 凍瘡の治療に有効ではないかと考えた。凍瘡患者42例に塩酸ジルチアゼム(Herbesser)1日量10∼120mg(分1∼分4)を連日投与し, 投与前後の臨床症状の改善, 血圧·脈拍の変動, 皮膚温の変化(ノバサーモNT-108型を使用), 組織学的変化を調べた。全例1ヵ月以内に治癒し, 副作用は1例も認められなかつた。また凍瘡の病態生理についても若干の検討を加えた。なお, 凍瘡治療にCa拮抗剤を用いた報告はこれが最初である。
