Abstract Background Surgical fundoplication is the standard treatment for refractory gastroesophageal reflux disease (GERD) with large (≥3 cm) hiatal hernias; however, its invasiveness is problematic in older adults and high‐risk patients. Anti‐reflux mucosal intervention (ARMI) is a less invasive endoscopic alternative; however, patients with large ...

Musculoskeletal (Mal)adaptations in Response to a > 30 000‐km Running Challenge
ABSTRACT Background Ultra‐endurance sports are increasingly popular, yet the long‐term physiological consequences of sustained extreme training loads remain poorly understood. In particular, the effects of prolonged ultra‐endurance exercise on skeletal muscle structure, function and molecular remodelling are largely unknown.

Dietary components shape the gut microbiome, which modifies bile acids through transformations. The resulting bile acid pool is sensed by host receptors to regulate immunity, metabolism, and intestinal function. Targeted bile acid measurements provide a unifying readout to study diet‐microbiome interactions and to develop next‐generation dietary ...

Slow‐transit constipation (STC) is a disabling motility disorder with unclear smooth‐muscle mechanisms. Spatial proteomic analysis of STC patient colon reveals both the central pathogenic role of smooth muscle cells (SMCs) in STC and novel regulators of intestinal motility, BIN1 and ALDH1B1.

MiR‐146a‐5p is downregulated in colorectal polyps, leading to IRAK1 upregulation and NF‐κB activation, which promotes M1 macrophage polarization and inflammation. Overexpression of miR‐146a‐5p suppresses IRAK1/NF‐κB signaling, reduces M1 polarization, and alleviates inflammatory responses.
