Effects of esaxerenone add‐on therapy versus angiotensin II receptor blocker dose escalation on albuminuria and blood pressure in hypertensive patients with type 2 diabetes: An exploratory randomized study

open access: yesJournal of Diabetes Investigation, EarlyView.
ABSTRACT Aims Residual albuminuria remains a major clinical challenge in patients with type 2 diabetes mellitus and hypertension despite renin–angiotensin system inhibitor therapy. Esaxerenone, a selective nonsteroidal mineralocorticoid receptor antagonist, reduces albuminuria; however, its effects on oxidative stress remain unclear.

Role of glucagon in metabolic diseases

open access: yesJournal of Diabetes Investigation, EarlyView.
ABSTRACT Currently, diabetes is defined as “a chronic hyperglycemic state resulting from the absolute or relative insufficiency of insulin action,” emphasizing its characterization as an insulin‐related disease. However, glucagon dysregulation should also play a critical role in the pathophysiology of type 2 diabetes.

Temporal distribution of acute kidney injury–related hospitalizations after initiation of sodium–glucose cotransporter 2 inhibitors: A nationwide claims database analysis in older adults with an independent complementary single‐center eGFR analysis

open access: yesJournal of Diabetes Investigation, EarlyView.
Among 190 older adults who experienced AKI‐related hospitalization after SGLT2 inhibitor initiation, 31.1% were hospitalized within 30 days. In an independent single‐center cohort, sustained early eGFR decline was associated with older age and use of one or two triple‐whammy‐related drug classes.

Frailty and low body mass index as key determinants of sodium‐glucose cotransporter 2 inhibitor‐specific adverse event‐related discontinuation in adults aged ≥75 years: A multicenter real‐world retrospective cohort study

open access: yesJournal of Diabetes Investigation, EarlyView.
Among Japanese adults aged ≥75 years newly initiating SGLT2 inhibitors, SGLT2 inhibitor–specific adverse event‐related discontinuation occurred in 9.1% within 180 days. Frailty and low BMI, rather than chronological age, were the key predictors, highlighting the importance of assessing physiological reserve before treatment initiation.

Do Intoxicated Offenders Deserve Harsher Sentences? Questioning Veritas in Vino

open access: yesJournal of Social Philosophy, EarlyView.
ABSTRACT Criminal courts increasingly treat intoxication as an aggravating rather than a mitigating factor in sentencing. This shift, seen in Australian law and other jurisdictions, raises the prospect of unjust outcomes. We examine this trend through the lens of desert‐based justifications for punishment, setting aside questions of deterrence and ...
