Endoscopic layngo-pharyngeal surgery (ELPS)
Toukeibu Gan, 2011ELPS（Endoscopic Laryngo-pharyngeal surgery）とは，彎曲型咽喉頭鏡で咽喉頭を展開し，内視鏡助手が軟性内視鏡で視野を提供しながら術者が硬性の直達鉗子類を用いて病変を切除する手術手技であり，良好な視野のもとに正確な領域診断をおこなって病変を切除することができる。これまで我々は，咽喉頭の未治療表在癌177病変（113例）と照射後残存・再発癌の5例にELPSを施行した。咽喉頭の未治療表在癌では，その疾患特異的5年生存率は87.5%，局所再発は2例であった。照射後の残存・再発例では，5例とも局所再発を認めなかった。偶発症・後遺障害としては，術後出血と術後瘢痕拘縮が課題として挙げられた。瘢痕拘縮の予防対策として，未治療表在癌2例のELPS後に，Fibrin ...
