COPD is a heterogeneous disease driven by overlapping neutrophilic and eosinophilic inflammatory pathways. This review highlights the shift from symptom‐based treatment toward phenotype‐ and biomarker‐directed care. Blood eosinophils, FeNO, and IgE predict corticosteroid responsiveness and identify candidates for biologic therapy.
