Analysis of equipment failures as contributors to chemical process accidents
Process Safety and Environmental Protection, 2013a b s t r a c t A database study of chemical process accident cases was carried out. The objective of the study is was to identify the reasons for equipment based accidents. The most frequent accident causing equipment were piping (25%), reactors and storage tanks (both 14%) and process vessels (10% of equipment accidents).

Decision Support System for Medical Equipment Failure Analysis
2018 9th Cairo International Biomedical Engineering Conference (CIBEC), 2018Medical equipment management raises a range of complex problems including those associated with maintenance process. In developing countries, hospitals rarely implement a coherent management plan in medical equipment management. One of the most significant challenges is to distinguish medical equipment that requires repair from those require ...

Hierarchy of Controls Analysis for Equipment Failures Prevention
2015Equipment failures play significant roles in most accidents that occur and recur in the chemical process industry. Many risk reduction strategies were applied to prevent these accidents but the accidents keep on happening. In this study, 50 equipment-related accidents are analyzed based on their comprehensive accident investigation reports, retrieved ...

Analysis and Prevention of Thermally Induced Failures in Electronic Equipment
1994This paper begins with a discussion of the physical phenomena associated with thermally-induced/accelerated failures, including diffusion-driven and thermo-mechanical failure mechanisms. Attention is then turned to the results of ongoing Northern Telecom “field-return” surveys and to the identification of the factors controlling device field ...

Analysis of the in-Service Failures of Metal Structures of the Aircraft Equipment
Materials Science, 2016The main causes of fractures of the aircraft structures made of steel, aluminum alloys, and titanium alloys are analyzed. On the basis of the experience of operation of “AN” aircrafts, we analyze the methods aimed at the prevention of fractures of metal parts and units of the aircraft equipment.

Study on Equipment Failure Based on Static Strength Analysis
Key Engineering Materials, 2018In this paper, the theory of process failure as the basis for analysis of working conditions based on the use of production line process equipment. In ANSYS static strength theory, technology and equipment to analyze the static strength, find the weak points of equipment. For weak points and system reliability and lifetime prediction, and establish the

Failure Mode Analysis of Mobile Equipment Hydraulic Leaks
Journal of Pressure Vessel Technology, 2012Leaks in general and hydraulic leaks in specific are one of the major failure modes for mobile equipment. In-plant leak rates may range from 1% to 25%, while field leaks may range from 1% to 13% depending on the complexity of the system, assembly process, and parts quality. In this paper, the main potential leak causes are discussed and the performance
