The extensor digitorum longus muscle (EDL) arises from the upper three-quarters of the extensor surface of the fibula, the interosseous septum and from a small area of the tibia across the superior tibiofibular joint. It forms its four tendons which are restrained by the superior and inferior extensor retinacula.

After tendon transfer of extensor indicis proprius (EIP) to extensor pollicis longus (EPL), rehabilitation is initiated to enhance motor cortex reorganization. However, patients have been described showing thumb extension immediately after the tendon transfer. At cortical level, no evidence supports either of these assumptions.

Transfer of the insertion of the extensor carpi ulnaris into the tendon of the extensor pollicis brevis may be used to restore opposition of the thumb. The natural insertion of the short extensor of the thumb has an ideal location for this purpose. It helps to bring the thumb into complete pronation when the force of pull is directed properly.
