A case of avascular necrosis of the femur head is presented. Its clinical picture is similar to that of osteoarthritis of the hip. The examination should include plain film radiography and radionuclide scan. When these two imaging procedures are equivocal, then magnetic resonance imaging should be considered.

Diagnosis of avascular necrosis (AVN) of the hip has been improved by the technical progress of imaging modalities during the last decade. For a long period, only plain radiographs had been available. Scintigraphy and computed tomography contributed to differential diagnosis and early detection of bone necrosis.

Aseptic necrosis of the femoral head appears to have a number of etiological factors. The last steps of the pathogenetic course of events, however, is unclear. Histologically, small foci of marrow necrosis form repeatedly, particularly in the upper and medial zone of charge, and then fuse secondarily to larger areas.
