Acute Pericardial Effusion After Vein of Marshall Ethanol Infusion for Persistent Atrial Fibrillation

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
This study reports a 25.2% incidence of acute pericardial effusion following vein of Marshall ethanol infusion for persistent atrial fibrillation. The extent of the resulting low‐voltage area was identified as an independent predictor of pericardial effusion, reflecting the magnitude of chemical injury.

Efficacy and Safety of Catheter Ablation in Very Elderly Patients With Atrial Fibrillation: Short‐ To Mid‐Term Outcomes From a Multicenter Study

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
Patients aged ≥ 80 years with atrial fibrillation underwent catheter ablation. At 2 years, the recurrence‐free survival rate was 85.8% in paroxysmal AF and 63.6% in persistent AF (p = 0.024), and periprocedural complications occurred in 12.0%, including severe complications in 3.3%.

Electro‐Mechanical Dissociation in Chagas Cardiomyopathy: Comparative Analysis of Arrhythmic Burden Beyond LVEF

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
Despite matched clinical characteristics and LVEF, patients with Chagas cardiomyopathy exhibit a significantly higher arrhythmic burden compared to ischemic cardiomyopathy. This electro‐mechanical dissociation highlights the necessity of sudden death risk stratification strategies that extend beyond traditional ejection fraction thresholds.

Efficacy of a Novel Ablation Index Algorithm for Cavotricuspid Isthmus Ablation Using a Flexible‐Tip Ablation Catheter

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
AutoMark Index (AI) is a novel ablation algorithm that combines contact force, radiofrequency current, and application to provide real‐time lesion formation. AI‐guided CTI ablation protocol is efficient in shortening RF duration for acute bidirectional CTI block and CTI procedure time compared to the conventional 40 W ablation protocol.

Sex‐ and Age‐Dependent Outcomes of Cryoablation in Atrial Fibrillation: A 3‐ and 6‐Month Post‐Procedural QoL Analysis

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
Cryoballoon ablation for Afib significantly improves overall QoL, PCS, and MCS. Notably, younger patients achieve substantially greater functional recovery across all components, whereas sex‐specific differences in these outcomes require further investigation.

Current Topics of Progressive Cardiac Conduction Disease

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
Many genes and the protein cause PCCD. Mutation of NaV1.5 or CX40 cause isolated PCCD, but mutation of lamin A/C, emerin, or desmin lead to cardiomyopathy, and PCCD. Mutation of transcription factor NCX2‐5, and Tbx5 associated with atrial septal defect and abnormal development of conduction system.

Clinical Outcomes of High Density Voltage and Fractionation Mapping‐Guided Ablation in Patients With Persistent Atrial Fibrillation

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
High‐density fractionation mapping‐guided ablation combined with voltage‐guided segmental pulmonary vein isolation improved 1‐year freedom from atrial fibrillation recurrence compared with conventional circumferential pulmonary vein isolation in patients with persistent atrial fibrillation, without increasing procedural complications or total ablation ...

Significance and Usefulness of High‐Resolution 24‐h Electrocardiographic Parameters in Patients With Coronavirus Disease 2019 After Admission: A Prospective Cohort Study

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
In patients with coronavirus disease 2019 (COVID‐19) without an obvious cardiac disease, ambulatory electrocardiographic markers can be useful in assessing the risk of severe disease. Particularly, simultaneous positivity of late potential (LP) and T wave alternans (TWA) in patients with COVID‐19 was a poorer prognostic factor than LP or TWA alone ...

Early Rhythm Versus Rate Control in Older Adults With New‐Onset Atrial Fibrillation: A Propensity Score–Matched Analysis

open access: yesJournal of Arrhythmia, Volume 42, Issue 3, June 2026.
In a large propensity score–matched real‐world cohort of older adults with newly diagnosed atrial fibrillation, early rhythm control was associated with lower thromboembolic and bleeding risk but higher mortality and arrhythmic complications compared with rate control.
