Structural MRI and 18F‐FDG‐PET reveal distinct structure‐metabolism decoupling in temporal lobe epilepsy. A presurgical machine learning model integrating these complementary radiomics with clinical features robustly predicts postoperative seizure freedom, achieving prognostic efficacy that could rival models reliant on actual resection data.
