Iliac Vein Injuries

Iliac vein injuries are highly lethal and present a substantial challenge to manage. They account for ~5% of cardiovascular injuries following trauma and are difficult to diagnose and repair due to their location deep in the pelvis. A high index of suspicion must be maintained to make a rapid diagnosis and have a chance at repair.

Iliac vein bypass with autogenous saphenous vein for iliac vein compression syndrome.

A case of chronic obstruction of the left iliac vein is presented. The patient had failed conservative management. Surgical reconstruction was performed using autogenous saphenous vein. The patient has done well; results of a venogram at six months postoperatively indicate the graft is still patent.

The coexistence of a retroperitoneal venous anomaly requires special consideration during open aortoiliac surgery. Anomalies of the external iliac vein are rare, and no anomalies of the right external iliac vein, which passes ventral to the external iliac artery, have been reported in the English literature to the best of our knowledge.

Linear calcification in the soft tissues of the pelvis in the region of the external iliac vein should suggest the possibility of calcified external iliac vein thrombosis, particularly in patients with a suspicious clinical history or physical findings.
