ABSTRACT In patients with moderate‐to‐severe COPD, short‐term (1‐day) preoperative respiratory optimization may be insufficient to prevent acute exacerbation after extubation. Prolonged preoperative preparation, meticulous intraoperative lung‐protective ventilation, and a post‐extubation rapid‐response protocol are essential.
