Pharmacokinetics of isosorbide-5-mononitrate in hemodialysis patients.

狭心症を有する慢性血液透析患者6名に対し, 硝酸薬であるisosorbide-5-mononitrate (ISMN) を朝夕1日2回各20mgを経口投与し, 3日以上経過後の透析・非透析日にそれぞれ時間採血を行い, 血中濃度の推移を検討した. 非透析日の血中濃度は高値であり, これには分布容積の減少が主因と考えられた.血液透析では平均血中濃度は約80%減少するが, 透析終了後も血中濃度は治療域に保たれていた. ISMNを慢性血液透析患者に投与する場合, 用量の補正等は必要ないと思われるが, 非透析日の血中濃度が高いこと, 透析の影響を受けることなどを考慮する必要があると考えられた.
