Hypernatremia and Lactulose Therapy

Lactulose is used in the treatment of portal-systemic encephalopathy. Although it is usually safe, a case of hypernatremia has been reported. To evaluate further the association of lactulose therapy and hypernatremia, records of patients with hepatic failure treated with lactulose for encephalopathy were analyzed.

The Rectal Administration of Lactulose

Summary: A lactulose colonic washout (pH 4.5) was administered to six male patients with portal systemic encephalopathy. Estimation of blood ammonia levels and electroencephalography were performed before and after each treatment. Five patients recorded a significant fall in blood ammonia level as a result of lactulose therapy (P < 025).

Twelve healthy volunteers were studied for two test periods, at the beginning of which they ingested a diarrheogenic load (60 g) of lactulose in 350 mL water with 10 g polyethylene glycol 4000 (PEG); the two periods were separated by a lactulose feeding period of 8 d, during which a nondiarrheogenic load (20 g) of lactulose was taken twice daily.

Abstract An improved colorimetric method is described for the estimation of lactulose in mixtures of lactose and lactulose. The anthrone method for the estimation of lactulose was modified by altering the concentrations of alcohol and acetic acid added to the reaction mixture.
