This study examines MPXV immunity in PLWH, focusing on smallpox vaccine‐induced responses. PLWH with CD4⁺ T‐cell counts <500 cells/mm3 showed reduced VTT‐IgG, MPXV‐IgG, and NAbs, while CD4⁺ T‐cell responses were impaired at counts <350 cells/mm3. CD8⁺ T‐cell responses remained stable.
