Neoadjuvant chemoradiotherapy (NCRT) significantly improves overall survival over adjuvant chemoradiotherapy for resectable cT3‐4a esophageal squamous cell carcinoma, with comparable progression‐free survival and toxicity. This propensity‐matched study supports NCRT as the preferred strategy for this high‐risk population.
