New indication standard of home oxygen inhalation therapy.

1985年にわが国で在宅酸素療法(HOT)が保険診療として認可されてからすでに10年の歳月が経つ.その間,在宅医療の充実や機器などの進歩とともにHOTの適応基準の見直しが図られてきた.適応基準の妥当性は厚生省特定疾患呼吸不全調査研究班の実態調査から明らかであるが,低酸素血症が生体に及ぼす様々な影響に関して得られた知見を全て反映しているわけではない.動脈血酸素分圧(PaO2)値を主眼におき, 60torrを基準とすることでこれらの要項の多くを満たせる.問題はPaO2が60torrより良好な症例でHOTを必要とするものの見極めである.現状では,個々の症例で肺高血圧,睡眠時低酸素血症,労作時息切れ,右心不全の既往歴などのマイナス因子を検討して, HOTやその他の併用療法の必要性を処方医が判断すべきである.
