Parathyroid Autotransplantation

7 patients with diffuse parathyroid hyperplasia underwent total parathyroidectomy and microsurgical autotransplantation for diffuse parathyroid hyperplasia. 5 of 7 patients showed good function of the grafted parathyroid tissue, as checked by the separate PTH assay from the grafted versus not grafted forearm.

Intrathyroidal Parathyroid

Fifty-eight human fetal thyroid glands obtained at autopsy were systematically studied for the presence of intrathyroidal parathyroid tissue. The latter was found in 13 thyroid lobes from 12 fetuses. It was located in a subcapsular position in 9 of 58 cases and lying deep in thyroid tissue in 4 of 58.

Although parathyroid neoplasms are common and cause primary hyperparathyroidism, parathyroid carcinoma is a rare entity. At times it can be difficult to diagnose. Patients with parathyroid carcinoma usually present with profound symptoms of hyperparathyroidism and highly elevated serum calcium and parathyroid hormone (PTH) levels.

Although rare following initial cervical exploration, reoperative parathyroid surgery may cause permanent hypoparathyroidism in 15% to 30% of patients. Immediate fresh or delayed cryopreserved parathyroid autotransplantation is the principal surgical option to resolve this complication.

It is important to consider hyperparathyroidism in the differential diagnosis of patents with hypercalcaemia. PARATHYROID CYSTS

Parathyroid cysts are relatively uncommon. They may be non‐functioning or associated with primary hyperparathyroidism. Three patients are presented, including one with a large mediastinal parathyroid cyst, and these cases illustrate the range of pathological and clinical features.
