The case history is reported here of a woman who has developed involuntary movements while having prolonged phenothiazine therapy. The disorder has developed gradually and is apparently irreversible. It has some unusual features.

Cardiovascular effects of phenothiazines

Abstract Phenothiazines have significant electrophysiologic and cardiocirculatory effects. The electrophysiologic effect of these drugs is similar to that reported for quinidine. Phenothiazines decrease the rate of rise of phase 0 of the action potential, decrease the duration and amplitude of phase 2, and prolong phase 3.

Mechanism for Phenothiazine Oxidation

The mechanism of phenothiazine degradation was studied by following the degradation of 3,10'-diphenothiazine in ethanol-water mixtures as well as the electrochemical oxidation of phenothiazine. A mechanism, including the formation of an oxidized dimer and some polymers, is suggested.
