Pseudothrombocytopenia Due to Agglutinins

Pseudothrombocytopenia may have any of a number of causes, one of which is agglutination in vitro. This phenomenon was found in samples of blood from six patients. A serum factor responsible for the agglutination was demonstrated. The factor was dependent upon the presence of EDTA and was more active at room temperature than at 37 C.

Psuedothrombocytopenia, the presence of platelet clumping on the peripheral blood film, is usually considered to rule out the diagnosis of immune thrombocytopenia (ITP) or other causes of increased platelet destruction and to be a laboratory artifact of no clinical significance. We report a case of concurrent psuedothrombocytopenia and ITP.

BACKGROUND: EDTA pseudothrombocytopenia (PTCP) is an in vitro artifact in which the anticoagulation of blood with EDTA is associated with in vitro agglutination of platelets, resulting in a spuriously low platelet count. In apheresis donors, whole‐blood samples for complete blood counts are routinely drawn into tubes anti‐coagulated with EDTA.

Pseudothrombocytopenia associated with vasculitis

A patient with pseudothrombocytopenia due to the anticoagulant EDTA is described. The thrombocytopenia was secondary to platelet agglutination and required the presence of EDTA in a critical concentration and the patient's plasma. Both homologous and autologous platelets were affected by this plasma factor in the presence of EDTA.
