A scant 10 years ago, the causes of recurrent attacks of acute pancreatitis in patients with no history of gallstones or heavy alcohol consumption would have remained undiagnosed and untreated. We now know that at least five other conditions should be considered and that most can be treated effectively.

Riluzole-induced recurrent pancreatitis
Journal of Clinical Neuroscience, 2017Riluzole is the only drug approved for the treatment of patients with Amyotrophic Lateral Sclerosis (ALS). It is well tolerated, being the most frequent adverse effects asthenia, nausea and reversible increase of liver enzymes levels. Severe adverse effects are extremely rare.

Intraluminal pancreatic candidiasis presenting as recurrent pancreatitis
Gastroenterology, 1993An 88-year-old man with closely spaced attacks of acute pancreatitis who was found to have ductal changes of chronic pancreatitis with multiple noncalcified intraluminal filling defects during endoscopic retrograde pancreatography is presented. These defects proved to be fungus balls made up of Candida albicans.

Recurrent acute pancreatitis is commonly seen in clinical practice and needs careful evaluation. Endoscopic therapies have a limited role. Genetic testing is not routinely done. Careful clinical evaluation and noninvasive testing should be performed first before doing more invasive evaluations/therapies. Well-designed clinical trials are needed.
