Fractures of the scaphoid can be classified into either undisplaced, stable fractures or displaced, unstable fractures by their roentgenographic appearance. When there is greater than 1 mm of fracture offset or an instability collapse pattern (dorsal lunate rotation) on the lateral view, an unstable, displaced fracture is present.

Articular Fractures of the Distal Scaphoid
Journal of Hand Surgery, 1988We propose an anatomical classification of fractures involving the distal articular surface of the scaphoid. The predominant patterns, in the 37 patients reviewed, were avulsion fractures from the radio-volar tip of the tuberosity and impaction fractures of the radial half of the articular surface. The possible mechanisms of injury are discussed.

Fractures of the Distal Radius and Scaphoid
Journal of Hand Surgery, 1996Simultaneous fractures of the distal radius and scaphoid is an uncommon wrist injury. During the period 1980 to 1993, 23 patients with a median age of 39 years were treated for this injury. The median follow-up period was 7 years. All scaphoid fractures were undisplaced and located in the waist or distal third.

Detection and treatment can be delayed due to only slight clinical symptoms, which are often shown by acute injuries of the scaphoid. Pseudarthrosis of the scaphoid developing arthrosis of the wrist or even a carpal collapse can be the consequence.
