Elevated cardiovascular risk associated with hormone replacement therapy: A comprehensive analysis of reproductive factors and atherosclerotic cardiovascular disease outcomes in the UK Biobank cohort

Among 170,250 UK Biobank women (2006–2024), ever HRT use was associated with higher incident ASCVD risk (adjusted HR 1.13, 95% CI 1.08–1.18), largely driven by ischemic stroke. Associations were stronger in women ≤45 years and during perimenopause. Mediation via sex hormones, inflammatory markers, or lipids was minimal (<2%).
