This study highlights the critical role of maintaining spino‐pelvic alignment in elderly patients with lumbar spinal stenosis. Postsurgical thresholds for PI‐LL (17.7°) and PT (26.1°) were associated with improved clinical outcomes, offering valuable benchmarks for surgical planning and postoperative care to enhance quality of life.
