Hospital-wide Rollout of Antimicrobial Stewardship: A Stepped-Wedge Randomized Trial [PDF]

open access: yesClinical Infectious Diseases, 2014
Our objective was to rigorously evaluate the impact of an antimicrobial stewardship audit-and-feedback intervention, via a stepped-wedge randomized trial. An effective intensive care unit (ICU) audit-and-feedback program was rolled out to 6 non-ICU services in a randomized sequence.

ATF4‐mediated stress response as a therapeutic vulnerability in chordoma

open access: yesMolecular Oncology, EarlyView.
We screened 5 chordoma cell lines against 100+ inhibitors of epigenetic and metabolic pathways and kinases and identified halofuginone, a tRNA synthetase inhibitor. Mechanistically halofuginone induces an integrated stress response, with eIF2alpha phosphorylation, activation of ATF4 and its target genes CHOP, ASNS, INHBE leading to cell death ...

Longitudinal exploration of the delivery of care following a successful antenatal practice change intervention

open access: yesFrontiers in Medicine
IntroductionThis is a secondary analysis of a stepped-wedge trial. The sustainment of evidence-based care is essential for ongoing population benefits.

Ethical Challenges of Randomized Violence Intervention Trials: Examining the SHARE intervention in Rakai, Uganda. [PDF]

open access: yes, 2016
ObjectiveWe identify complexities encountered, including unanticipated crossover between trial arms and inadequate 'standard of care' violence services, during a cluster randomized trial (CRT) of a community-level intimate partner violence (IPV) and HIV ...

Developing evidence‐based, cost‐effective P4 cancer medicine for driving innovation in prevention, therapeutics, patient care and reducing healthcare inequalities

open access: yesMolecular Oncology, EarlyView.
The cancer problem is increasing globally with projections up to the year 2050 showing unfavourable outcomes in terms of incidence and cancer‐related deaths. The main challenges are prevention, improved therapeutics resulting in increased cure rates and enhanced health‐related quality of life.

Evaluation of interventions to prevent vasovagal reactions among whole blood donors: rationale and design of a large cluster randomised trial

open access: yesTrials, 2023
Background Vasovagal reactions (VVRs) are the most common acute complications of blood donation. Responsible for substantial morbidity, they also reduce the likelihood of repeated donations and are disruptive and costly for blood services. Although blood

Sample Size Calculation for an Individual Stepped‐Wedge Randomized Trial

open access: yesBiometrical Journal
ABSTRACTIn the individual stepped‐wedge randomized trial (ISW‐RT), subjects are allocated to sequences, each sequence being defined by a control period followed by an experimental period. The total follow‐up time is the same for all sequences, but the duration of the control and experimental periods varies among sequences. To our knowledge, there is no

Stepped wedge trials with continuous recruitment require new ways of thinking [PDF]

open access: yesJournal of Clinical Epidemiology, 2019
There is substantial variation in the design of stepped wedge trials. Many recruit participants continuously over time, although the methodological literature has tended not to differentiate closely between continuous recruitment and discrete sampling.

Cell surface interactome analysis identifies TSPAN4 as a negative regulator of PD‐L1 in melanoma

open access: yesMolecular Oncology, EarlyView.
Using cell surface proximity biotinylation, we identified tetraspanin TSPAN4 within the PD‐L1 interactome of melanoma cells. TSPAN4 negatively regulates PD‐L1 expression and lateral mobility by limiting its interaction with CMTM6 and promoting PD‐L1 degradation.

Designing stepped wedge trials to evaluate physical activity interventions in schools: methodological considerations

open access: yesInternational Journal of Behavioral Nutrition and Physical Activity
Background In a stepped wedge design, schools are randomised to a sequence of measurements, with each sequence transitioning to intervention status at a different time.
