Endovascular Management of Acute Ischemic Strokes with Tandem Occlusions

Cerebrovascular Diseases, 2016
Background: Ischemic strokes with tandem occlusions are associated with a poor prognosis. Recent studies demonstrating the effectiveness of endovascular treatment for large vessel occlusions have shown less impressive results in patients with tandem occlusions than in those with isolated intracranial occlusions ...

Tandem Occlusion Causing Acute Ischemic Stroke

2019
Tandem occlusion (TO) is defined as the presence of concurrent large intracranial and extracranial artery occlusion causing an acute ischemic stroke (AIS). The stroke mechanism is usually an artery-to-artery embolization of a thrombus formed at the site of cervical carotid obstruction, which may be caused by atherosclerotic disease or dissection.

Coaxial Method of PTA Balloon and Aspiration catheter using the delivery wire of a stent retriever for tandem occlusion: CoMBAt tandem occlusion

Interventional Neuroradiology
Purpose Tandem occlusion, in which the intracranial artery is obstructed along with proximal carotid occlusive or stenotic lesions, occurs in 10–15% of patients with large vessel occlusion. Our study introduces a novel technique for tandem occlusion treatment that involves the simultaneous coaxial deployment of ...

Impact of Sex on Clinical Outcomes of Tandem Occlusion in Acute Ischemic Stroke Patients Treated With Mechanical Thrombectomy. A Propensity‐Matched Analysis [PDF]

open access: yesEuropean Journal of Neurology
Background: Although mechanical thrombectomy (MT) represents the standard of care for ischemic stroke due to large-vessel occlusion (LVO), the impact of sex on outcomes in tandem occlusions remains unclear.

Endovascular treatment of tandem vascular occlusions in acute ischemic stroke

Journal of NeuroInterventional Surgery, 2014
Background and purposeTandem vascular occlusions are an important cause of acute ischemic stroke (AIS) and present unique treatment challenges. We report our experience of managing a subset of AIS patients with extracranial vascular stenting/angioplasty and intracranial revascularization.MethodsConsecutive patients who presented at three centers with ...

Is Intravenous Thrombolysis Useful in Carotid Artery Dissection With Tandem Occlusion?

Journal of Stroke and Cerebrovascular Diseases, 2012
Cervical artery dissection no longer represents an obstacle for performing intravenous (IV) thrombolysis. This treatment can be used safely for patients with cervical artery dissection, but tandem lesions may be related to poor outcome. We report a patient with an occlusive internal carotid artery (ICA) dissection and middle cerebral artery (MCA ...

Mechanical thrombectomy in tandem occlusion: procedural considerations and clinical results

Neuroradiology, 2014
Acute tandem occlusions of the cervical and distal internal carotid artery (ICA) or middle cerebral artery (MCA) are associated with major stroke with intravenous (i.v.) thrombolysis alone in approximately 90 % of patients. The data on endovascular management of tandem occlusions is still limited.

[Tandem occlusions in acute ischemic stroke].

Der Radiologe, 2019
Acute strokes caused by tandem occlusions include an atherosclerotic or dissective stenosis/occlusion of the extracranial internal carotid artery (eICA) in combination with an intracranial vessel occlusion.Endovascular treatment can be technically challenging but is definitely superior to intravenous thrombolysis alone and achieves good clinical ...

Tandem Occlusions (Antegrade First Technique with Stent)

2019
A 70-year-old male was admitted with 5 h duration of left-sided weakness. The NIHSS score was 17, and the patient had a tandem occlusion; there was a tight stenosis at the origin of the right ICA along with M1 MCA occlusion. There were good collaterals along with a small infarct core (Fig. 90.1).
