Summary The carcinogenic effect of ionizing radiation on the human thyroid gland seems to be well established by studies involving a variety of exposed populations. The large ABCC series of occult thyroid carcinomas has indicated some interesting features of those lesions in addition to the radiation relationships.

Thyroid carcinomas are fairly uncommon and include disease types that range from indolent localised papillary carcinomas to the fulminant and lethal anaplastic disease. Several attempts to formulate a consensus about treatment of thyroid carcinoma have resulted in published guidelines for diagnosis and initial disease management.

Although thyroid carcinoma is relatively uncommon, approximately 33,550 new cases will be diagnosed in the United States in 2007. It occurs 2 to 3 times more often in women than in men, and with the incidence increasing by 4% per year, it is currently the eighth most common malignancy diagnosed in women.

Dr. Decosse: During the past ten to 20 years, there has been a notable evolution in the diagnosis and management of the more differentiated forms of carcinoma of the thyroid. No single contribution stands out as having redirected the course of therapy of patients with these tumors, but, from many different studies, gradual trends have emerged.

Medullary thyroid carcinoma (MTC) accounts for 5-8% of all thyroid cancers. MTC is mainly sporadic in nature, but an hereditary pattern [multiple endocrine neoplasia type 2 (MEN 2)] is present in 20-30% of cases, transmitted as an autosomal-dominant trait due to germline mutations of the RET proto-oncogene.

When is medullary thyroid carcinoma “medullary thyroid carcinoma”?
World Journal of Surgery, 1986AbstractMedullary thyroid carcinoma (MTC) is thought to develop from the parafollicular or C cells within the thyroid gland and, therefore, should not take up radioactive iodine. Recently, however,131I uptake has been reported in metastases of “medullary carcinoma” and, consequently, some authors have suggested that these tumors can be treated with ...
