A Meaning to Teach TIVA/TCI to Anesthesia Residents —A Current Condition to Perform TIVA or Use TCI—
プロポフォールTIVA/TCIの使用実態調査を行った. その結果, 手術の種類にもよるが開胸手術, 心臓外科手術ではTIVA施行頻度が50%以上であった. TIVAを躊躇する理由は 「吸入麻酔の方が調節しやすい」 と 「吸入麻酔の方が使い慣れている」 が多かった. また, 覚醒遅延の経験もTIVAを躊躇させていた. 日本麻酔科学会の教育ガイドラインによると, プロポフォールによるTIVAの行動目標について, 「プロポフォールを用いて全静脈麻酔ができる」 と記載されていることから, 研修医教育を行ううえでプロポフォールTIVA/TCIは必須である. 当施設のTIVA/TCI教育の実際を示し, それにより得られる効果を考察した.
