Use of cuffed anastomosis in total aortic arch replacement

The Annals of Thoracic Surgery, 2003
The distal aortic anastomosis portion of the total arch surgery remains technically complex especially in cases in which an aortic arch aneurysm extends below level of carina. We present the cuffed anastomosis that overcomes this difficulty. We applied this technique in 49 patients of elective total aortic arch aneurysm repair using selective cerebral ...

Mortality and Morbidity After Total Aortic Arch Replacement

The Annals of Thoracic Surgery, 2014
Total aortic arch replacement is associated with considerable mortality and morbidity. Although operative death is the most extreme adverse clinical end point, postoperative morbidity can also be devastating for survivors. We examined the short-term and long-term outcomes of 146 patients who underwent total aortic arch replacements between September ...

Total Endovascular Aortic arch Replacement with Chimney/Sandwich Techniques

Annals of Vascular Surgery, 2020
Traditional open total zone 0 replacement of the aortic arch is one of the most complex, challenging, and demanding operative procedures in cardiovascular surgery, associated with significant morbidity (30-40%) and mortality (8-20%). Total endovascular zone 0 replacement of the aortic arch with chimney/sandwich techniques as described by Lobato and ...

[Total Aortic Arch Replacement after a Prior Ascending Aortic Replacement].

Kyobu geka. The Japanese journal of thoracic surgery, 2021
Surgeries for non-dissected aortic arch diseases have been performed with increasing frequency in recent years. Accordingly, the awareness of the pitfalls in reoperation after a previous ascending aortic replacement is needed. Indications for redo total aortic arch replacement after a prior ascending aortic replacement include pseudoaneurysm formation ...

[Total aortic arch replacement for distal aortic arch aneurysms].

Kyobu geka. The Japanese journal of thoracic surgery, 1998
Between January 1991 to April 1997, 13 consecutive patients underwent the total aortic arch replacement for distal aortic arch aneurysms. The operation was performed through the median sternotomy and the cardiopulmonary bypass was instituted with the cannulation of ascending aorta for arterial perfusion and of right atrium for venous return.

Simple and Reliable Distal Anastomosis for Total Aortic Arch Replacement

Asian Cardiovascular and Thoracic Annals, 2008
In total aortic arch replacement, distal aortic anastomosis is often remarkably difficult because of the deep operative field. Once bleeding from the anastomotic area occurs, it is intractable not only because of technical problems but also decreased coagulability due to deep hypothermia and the fragility of the aortic wall.

Total Endovascular Aortic Arch Replacement: Branched Arch Endografts

2019
Aortic arch pathology has traditionally been approached through open repair necessitating circulatory arrest. Although a durable result, emerging technologies have been supplanting open technique, providing surgeons with a wider array of treatment options. These technologies include hybrid repair (debranching the aortic arch followed by stent grafting)

Selective Left Subclavian Ligation in Total Aortic Arch Replacement

The Annals of Thoracic Surgery, 2012
The left subclavian artery (LSA) is usually difficult to manipulate in total aortic arch replacement procedures if it is displaced by huge false lumens in the ascending aorta or right hemiarch. We summarize our experience of selectively ligating the deeply located LSA in total aortic arch replacement and stented "elephant trunk" implantation procedures

Total Arch and Descending Thoracic Aortic Replacement by Left Thoracotomy

The Annals of Thoracic Surgery, 2012
The hybrid treatment of transverse aortic arch pathologies with supraaortic debranching and endovascular repair is associated with significant morbidity and death and lacks long-term follow-up. The traditional two-stage open surgical approach to extensive arch and descending thoracic aneurysms carries a significant interval mortality rate.

Total aortic arch replacement through the L-incision approach

The Annals of Thoracic Surgery, 2003
Even though the median sternotomy is the standard approach for surgery involving the aortic arch, access to the site of distal anastomosis is problematic when the aortic pathology involves the distal arch. We recently developed an "L-incision" approach (a combination of a left anterior thoracotomy and upper half median sternotomy) for total arch ...
