Late Local Recurrence after Neoadjuvant Therapy and Radical Resection for Locally Advanced Rectal Cancer

open access: yes
Neoadjuvant radiochemotherapy (RCT) and lately total neoadjuvant therapy (TNT) improved local recurrence rates of rectal cancer significantly compared to total mesorectal excision (TME) alone. Yet the occurrence and impact of late local recurrences after

MPP8 Facilitates DNA DSB Repair via NHEJ to Promote Chemoresistance of TNBC Independent of Methyl‐H3K9 Binding

open access: yesAdvanced Science, EarlyView.
Chemoresistance is often linked to elevated DNA repair. MPP8 is found to promote breast cancer chemoresistance as a novel NHEJ accessory factor independent of its epigenetic functions. Through multiple protein interactions, MPP8 facilitates association of DNAPK and ligase complexes to promote DNA repair progression.

Lactate‐H3K18la‐RPL11 Axis Drives Gemcitabine Resistance in Pancreatic Cancer by Integrating Protein Synthesis and DNA Damage Repair

open access: yesAdvanced Science, EarlyView.
A lactate‐H3K18la epigenetic axis upregulates RPL11 to drive gemcitabine resistance in pancreatic cancer. RPL11 orchestrates chemoresistance through a dual, compartmentalized role: enhancing cytoplasmic protein synthesis via EEF1A1 stabilization and promoting nuclear DNA damage repair by co‐activating PARP14, thus ensuring robust cell survival ...

Current Status and Challenges of Imaging‐Based Diagnosis of Lymph Node Metastasis in Colon Cancer: The Lymph Node Committee of the Japanese Society for Cancer of the Colon and Rectum

open access: yesAnnals of Gastroenterological Surgery, EarlyView.
This review of the status and challenges of diagnosis of lymph node metastasis in colon cancer suggests that it is currently difficult to establish strict criteria for lymph node metastasis, indicating only moderate diagnostic performance. ABSTRACT Background Accurate pre‐treatment evaluation of lymph node metastasis (LNM) in colon cancer is important ...

Redefining the Role of Splenectomy in Scirrhous Gastric Cancer: Toward a Selective Multimodal Strategy

open access: yesAnnals of Gastroenterological Surgery, EarlyView.
ABSTRACT Scirrhous gastric cancer, characterized by diffuse infiltration and a high propensity for peritoneal dissemination, remains one of the most aggressive subtypes of gastric cancer. Despite a relatively high incidence of splenic hilar lymph node (No. 10) metastasis, the oncologic benefit of splenectomy remains controversial, while its association

Carcinoembryonic Antigen Messenger RNA in Pelvic Lavage Fluid Predicts Local Recurrence After Curative‐Intent Resection of Rectal Cancer

open access: yesAnnals of Gastroenterological Surgery, EarlyView.
Pelvic lavage carcinoembryonic antigen (CEA) mRNA was quantified in 178 patients undergoing curative‐intent resection for rectal cancer. Positive pelvic lavage CEA mRNA was significantly associated with local recurrence and provided additional prognostic information beyond circumferential resection margin (CRM) status.

Impact of Histopathological Response on Outcomes After Surgical Resection Following Carbon‐Ion Radiotherapy for Pancreatic Cancer With Arterial Involvement

open access: yesAnnals of Gastroenterological Surgery, EarlyView.
ABSTRACT Aim Carbon‐ion radiotherapy (CIRT) provides superior dose distribution and higher biological effectiveness than conventional X‐ray radiotherapy and has emerged as a promising component of multidisciplinary treatment for advanced pancreatic ductal adenocarcinoma (PDAC).

Local Excision in the Era of Total Neoadjuvant Therapy

open access: yes
Total neoadjuvant therapy (TNT) for rectal cancer has allowed for organ preservation in cases of complete response. At first, organ preservation was accomplished by a nonoperative approach that avoided proctectomy altogether.

Impact of Postoperative Infectious Complications on Long‐Term Survival After Curative Resection for Colorectal Cancer: A Multicenter Study in the Minimally Invasive Era

open access: yesAnnals of Gastroenterological Surgery, EarlyView.
Postoperative infectious complications (PICs) have been associated with poor long‐term outcomes after colorectal cancer surgery, but evidence in the modern minimally invasive era remains limited. In this multicenter cohort of 5377 patients undergoing curative resection, PICs were independently associated with worse overall survival and relapse‐free ...
