Hereditary Urticaria Pigmentosa

The second genetically significant report of urticaria pigmentosa in closely related individuals is presented. The hereditary pattern seems to be that of an incompletely penetrant autosomal dominance.

Urticaria pigmentosa (UP), resulting from the accumulation of excessive numbers of mast cells in the skin, is the most common form of cutaneous mastocytosis. Observations highlight the diversity of this disease. Clonal expansion of early hematopoietic progenitor cells carrying activating mutations in KIT seems to be the basis of adult-onset UP.

Despite numerous reports of urticaria pigmentosa, the riddle of the mast cell and the etiology of this disease have not been solved. Paradoxically, although extensive lesions with systemic involvement have been described, the disease has, with few exceptions 1-9 run a benign course.

Familial Urticaria Pigmentosa

We describe urticaria pigmentosa in three generations of a kindred. Unique to this family is the marked diversity of their skin disease. Classic lesions of urticaria pigmentosa, telangiectasia macularis eruptiva perstans, and clinically normal skin with positive dermal infiltrates of excess mast cells were found in individual family members.

Urticaria pigmentosa, characterized by cutaneous mast cell proliferation, usually occurs sporadically in childhood and resolves over a period. Rarely is the condition seen in two or more members of a family. We encountered a family in which urticaria pigmentosa occurred in four members of two generations. Skin findings were similar in all four.

Die Urticaria pigmentosa ist eine urtikarielle Erkrankung, welche unmittelbar nach der Geburt oder spatestens im ersten Lebensjahre beginnt und die von der Urticaria cum pigmentatione streng zu scheiden ist (s. Urticaria). In einzelnen Fallen ist sogar intrauteriner Beginn beschrieben worden.

An Hand mehrerer Falle wird das pathologisch-anatomische Substrat der Urticaria pigmentosa im Sinne der Unnaschen Definition entwickelt und darauf hingewiesen, das diese Diagnose niemals ohne entsprechende histologische Untersuchung gestellt werden sollte. Von der Urticaria pigmentosa werden die Formen der Urticaria eindeutig abgegrenzt.
