Working Knowledge for Fluid Management
麻酔中の輸液は血管内輸液,間質への輸液,細胞内への輸液の3本立てで考える.細胞外液製剤はブドウ糖以外の糖を浸透圧調整のため含んでいる製剤が容量効果と利尿効果をもっているので使いやすい.ヒドロキシエチルスターチ(HES)は血管内輸液として適切である.膠質浸透圧は測定膜,内皮細胞臓器特異性,病態を考慮する必要がある.HESには抗炎症作用があり,末梢循環を維持することができる.6% HES70は腎や止血凝固への影響は最も少ない.輸液のモニターとして尿量が臨床的である.圧モニターとしてのCVPより経食道心エコー下の容量負荷を重視した考え方がある.混合静脈血酸素飽和度や乳酸値は輸液を含めた周術期管理の総合的なモニターとして重要である.
