【目的】探讨脑梗死后再发缺血性卒中与脑出血患者的临床特点、影像学上微血管病变及预后的异同。【方法】连续收集急性复发性卒中的住院病例(初发事件为脑梗死,复发事件包括脑梗死或脑出血),详细记录临床资料。观察脑内微出血及自质病变的严重程度,随访有无严重血管事件及死亡的发生。【结果】175例再发脑梗死患者。19例复发事件为脑出血的患者进入本研究。155例(88.6%)再发脑梗死患者,以及18例(94.7%)脑出血患者存在自质病变。77例(44%)再发脑梗死患者和16例(84.2%)脑出血患者存在脑内微出血 ...

1985-1992年,作者收治脑室出血35例。单纯脑室出血6例,脑实质出血破入脑室29例。一侧或二侧脑室出血22例,二侧脑室合并三脑室或三、四脑室出血13例。出血原因:高血压25例,动静脉畸形4例,脑血管瘤1例,原因不明5例。依脑室出血范围及血肿量多少采用单侧或双侧钻孔引流。引流时间2-25d。引流量60-380ml/d。经治疗存活22例,恢复原来工作或生活自理8例,生活半自理12例,在死亡13例中,术后合并肺炎、上消化道大出血9例。临床研究结果提示,脑室引流是抢救本病有效的重要措施。

本文对365例不同原因的脑血管病变尸检病例作了分析讨论。高血压动脉硬化占全部病例的26.85%。平均年龄58.4岁。脑的病变包括出血、出血伴梗塞及单纯梗塞。其中脑出血最多,但随着脑动脉粥样硬化程度加重,脑出血比例相对地减少,脑出血加梗塞及脑梗塞相对增加。
