脓毒症是宿主对感染反应失调导致危及生命的器官功能障碍,是重症病人主要的死亡原因。脓毒症确切的发病机制不明,免疫抑制可能是主要的致病因素之一。免疫抑制导致脓毒症患者无法抵抗原发性细菌感染和容易发生继发感染,造成脓毒症患者住院时间延长和死亡增加,识别免疫功能抑制并给予干预将会使这部分患者获益。本文通过临床高危群体筛查和特征性免疫标志物检测两个角度帮助临床医生识别免疫抑制的患者,并介绍目前已经进入临床或临床试验阶段的免疫增强的药物。

预测输尿管软镜碎石术后并发尿源性脓毒症的反向传播神经网络模型构建 [PDF]

目的构建输尿管软镜碎石术(FURL)后并发尿源性脓毒症的反向传播神经网络预测模型。方法纳入428例接受FURL的肾结石患者,根据术后是否并发尿源性脓毒症分为脓毒症组(42例)和对照组(386例)。采用logistic回归分析确定FURL后并发尿源性脓毒症的影响因素及其交互作用。同时建立logistic回归模型和神经网络模型进行预测,通过受试者工作特征曲线评估两种模型的预测效能。结果单因素分析显示,结石手术史、性别、尿培养阳性、结石直径、糖尿病、手术时间、白细胞、血小板、C反应蛋白(CRP)及肝素结合蛋白(
