Clinical use of anti-HIV agents
現在の抗HIV療法は,カクテル療法と言われるように3剤以上の多剤併用で行うことが基本である.国内で承認されている抗HIV薬は,その作用機序から,逆転写酵素阻害薬とプロテアーゼ阻害薬に分類され,逆転写酵素阻害薬は更に,核酸系と非核酸系とに分かれる.一般的には,非核酸系逆転写酵素阻害薬か,または,プロテアーゼ阻害薬をキードラッグとして,それに,2剤の核酸系逆転写酵素阻害薬を組み合わせて投与する.それぞれの薬剤の作用機序,特性,副作用を認識し,相互作用を考慮しつつ,組み合わせて投与することとなる.副作用は,同じ作用機序の薬剤に一般的に見られるものと,薬剤特異的なものがあり,注意が必要である.既に承認されている抗HIV薬の組み合わせのみではコントロール不良な多剤耐性HIVを持つ症例もあり ...
