A patient with resectable NSCLC underwent neoadjuvant pembrolizumab‐based chemoimmunotherapy and lobectomy, which unexpectedly revealed extensive amyloid deposition in the lung and lymph nodes. LC–MS/MS identified AL (λ) amyloid, and hematologic workup confirmed an underlying λ‐restricted plasma cell neoplasm with t(11;14).
