AGING MEDICINE, Volume 8, Issue 1, February 2025.This study evaluates a multidisciplinary approach to managing polypharmacy in community‐dwelling older adults. Over 3 months, patients experienced a significant reduction in medication load, from an average of 9.6–8.9 medications, and a decrease in STOPP criteria and anticholinergic burden.
