Antigen Report: HLA-B27

As in the previous Workshops, HLA-B27 appears to be one of the best defined antigens. Four sera are very good or excellent (Table 1) as, indicated by their quality scores based on the formula of Mickey and Terasaki. The few extra reactions of the above-mentioned sera are cross-reactions, mostly with B7.

Microbial antigens mediate HLA-B27 diseases via TLRs

HLA-B27 positive individuals are predisposed to reactive arthritis developing 1-3 weeks after urogenital and gastrointestinal infections. Also ankylosing spondylitis (AS) associates strongly to HLA-B27, but no specific infection, Klebsiella pneumoniae excluded, has been linked to it.

Association of HLA-B27 Antigen with Retroperitoneal Fibrosis

This is the second known case of retroperitoneal fibrosis in a middle-aged black man whose tissue type was HLA-B27 positive. The HLA-B27 antigen is present in only 4 per cent of black American male subjects and may be important in the diagnosis and management of patients with primary retroperitoneal fibrosis.

HLA-B27 antigen in women with ankylosing spondylitis.

Twenty-three (85.2%) of 27 female patients with ankylosing spondylitis were positive for HLA-B27 antigen, including 15 of 18 white women (83.3%) and eight of nine black women (88.9%). Ankylosing spondylitis in women appears to have the same high association with HLA-B27 antigen as that reported in predominantly male studies (88% to 96%).
