レジオネラは土壌細菌で世界中に分布しているが,近代の文化的生活における人工環境の至る所で増殖し,近年新たな展開を見せている.特に,人工環境水においては共存するアメーバを媒体として大量に増殖し,ありとあらゆる環境水からレジオネラ感染の危険性が高まっている.人工環境水の消毒の問題は人体への影響を考慮する必要があり,困難をきわめている状態である.わが国にはビル管理基準としての冷却塔におけるレジオネラ症防止指針が作成されているが,米国のCDCのような院内感染防止指針やその他の環境水に対する水の管理基準の策定はこれからである.診断法はPCR法や尿中抗原検出などかなりの進歩が見られているので,疑わしい症例では積極的にしかるべき施設へ依頼してでも確定診断を得る努力をする必要がある.
