インスリンあるいはインスリン受容体の異常が糖尿病者で見い出された.インスリン受容体に結合しえない異常インスリンやプロインスリンが産生されるインスリン遺伝子の異常,細胞表面のインスリン受容体の数や機能が減少するインスリン受容体遺伝子の異常あるいはインスリン受容体に対する自己抗体の存在が報告されている.いずれも高インスリン血症の存在が診断への手掛りとなるが,インスリンの異常ではインスリン抵抗性は存在しないがインスリン受容体の異常では存在する点が最大の鑑別点となる.インスリン遺伝子あるいはインスリン受容体遺伝子の異常が見い出された家系について検討すると,糖尿病の発症年齢,重症度は均一でなく,その発症には他の遺伝子異常あるいは環境要因の関与も推定され,糖尿病の発症にはいくつかの因子が関与していると考えられた.
