Ischemic heart disease in the elderly.
老年者の虚血性心疾患は冠状動脈自体の動脈硬化性病変に加え心臓の生理的老化や他臓器障害など複雑に関与している. 急性心筋梗塞の臨床像, 選択的冠状動脈所見, 病理学的検索の3面から老年者の虚血性心疾患の特徴について検討した. 急性心筋梗塞例中70歳以上の老年者72例を非老年者128例と対比した. 老年者群の特徴は加齢とともに女性の占める率が増加し, 梗塞部位では心内膜下梗塞が24%と高率であった. 冠状動脈硬化危険因子中高脂血症, 肥満, 喫煙は有意に低率であった. 心不全, 心室頻拍・細動, 心房細動の合併率は有意に高く, 死亡率は25%で, 非老年者群の13%に比し有意に高かった.選択的冠状動脈造影で75%以上の有意の狭窄を呈した65歳以上の81例を40歳以下の若年者群と対比検討した ...
