• We analysed 378 RCTs (113 122 participants). • Dual GLP‐1/GIP agonists reduced T2D risk by 90%, followed by GLP‐1 agonists (74%) and metformin plus ILI (73%). • Physical activity and ILI improved glycaemic markers at 12 months. Lifestyle benefits peaked at 3–6 months, whereas metformin effects appeared longer‐lasting. • Lifestyle modifications remain
