膠原線維性大腸炎(collagenous colitis,以下CC)は1976年にLindstromによって最初に報告された水様性下痢を主徴とする大腸の炎症性疾患である.その後欧米を中心に同様の臨床症状を呈するリンパ球性大腸炎(lymphocytic colitis,以下LC)の概念も提唱され現在では両者とも広く認められている.両者は慢性の水様性下痢を主訴として発症し,大腸内視鏡や注腸造影で明らかな異常所見に乏しく,生検によって確定診断がなされるとされている.その病理学的特徴はCCでは粘膜上皮直下の厚いcollagen bandの証明,LCにおいては粘膜上皮内のリンパ球の増加を証明することによってなされる.本邦では少数の症例報告が見られるのみであるが,今後本症の疾患概念が広く認識され ...
